Fill in and return this form with the following supporting documents to the school :

• A copy of Birth Certificate or extract
• Immunisation Records
• Copies of Family Court or any other court orders (if applicable).
• Proof of address (copy of bills, statements, lease agreements etc.)
• Information relating to suspensions or exclusions
• Copies of at least 2 of the most recent school reports
• Copy of most recent NAPLAN results and WAMSE results
• Information relating to any disability or diagnosed condition
